Please fill out this registration form and send it in with payment. Upon receipt of your payment, your spot in the class is reserved. If by some odd chance, the class cancels and doesn't happen, your check will be returned to you. If you can't make it to the class and notify me at least a week prior to the date of the class, your payment less 10% will be returned.
